BEFORE MAKING A REFERRAL
Professional referral criteria and service boundaries
Who may be referred?
Bluebell Haven Care Ltd currently considers referrals involving adults aged 18 or over in Crawley, Horsham and nearby West Sussex communities.
The proposed support must fall within the relevant Bluebell Haven Care pathway, and the adult or their authorised representative must be able to take part in the discussion and decision-making process.
What can a Home Support referral include?
Home Support may include companionship, shopping and errands, simple meal preparation, light household tasks, community access and support with everyday routines.
The Home Support service does not include regulated personal care, nursing, medication administration or clinical treatment.
What can a Personal Assistant introduction referral include?
Bluebell Haven Care Ltd may help define the proposed Personal Assistant role, search for potentially suitable candidates, carry out agreed pre-introduction screening and provide relevant candidate information.
The individual or their authorised representative decides whom to meet and choose and remains responsible for directing and managing the ongoing arrangement.
What authority or agreement is needed before referring?
The referring professional or organisation must have an appropriate basis and authority to share the information submitted.
The adult should know about the referral and understand how their information will be used where this is appropriate. Different rules may apply where information must be shared to protect someone from serious harm or meet a safeguarding responsibility.
What information should be shared initially?
Provide only the information needed to understand the proposed referral, including the general support sought, location, preferred times, communication requirements and the reason for the enquiry.
Do not submit medical records, care plans, NHS numbers, medication lists, identity documents, financial records or detailed diagnoses through the website form. We will explain a more appropriate process if further information is genuinely required later.
Which referrals are not suitable for this page?
Do not use this page for an emergency, crisis intervention, urgent medical treatment or a statutory safeguarding referral.
The page is also not an appropriate referral route for nursing, clinical treatment, unmanaged regulated personal care or services for children.
Does submitting a referral guarantee acceptance?
No. Every referral is reviewed individually.
Acceptance depends on the person’s circumstances, the requested service, location, proposed timing, service boundaries, information-sharing arrangements and current workforce or candidate capacity.

PROFESSIONAL REFERRAL QUESTIONS
Important information for referrers and partner organisations
Is this a statutory or clinical referral service?
No. This is an initial enquiry route for Bluebell Haven Care Ltd’s current support, introduction, staffing and partnership pathways.
It does not replace statutory social care assessments, NHS clinical referrals, emergency services or local authority safeguarding procedures.
Can a professional refer someone for regulated personal care?
Bluebell Haven Care Ltd’s Home Support service does not include regulated personal care, nursing, medication administration or clinical treatment.
A Personal Assistant introduction may involve a proposed role that includes personal care only where the individual or representative chooses and manages the PA directly and Bluebell Haven Care has no ongoing direction or control over the arrangement.
Can I submit a referral without the adult knowing?
The referring organisation is responsible for identifying an appropriate lawful and professional basis for sharing information.
Where appropriate, the adult should know about the referral, understand why information is being shared and know where to find Bluebell Haven Care Ltd’s Privacy Policy. Safeguarding and emergency situations may require a different approach.
Will Bluebell Haven Care contact the adult directly?
Where appropriate and agreed, we may contact the adult or their authorised representative to understand their preferences and explain the proposed pathway.
We will not assume that a professional referral overrides the adult’s choices or commits them to a service.
Can urgent or emergency referrals be submitted?
Do not use the professional enquiry form as an emergency or crisis referral route.
Call 999 if someone is in immediate danger. Use the appropriate statutory safeguarding, NHS or local authority route where urgent action is required.
Can referrals be accepted outside Crawley or Horsham?
We may consider enquiries from nearby West Sussex communities.
Availability depends on the postcode, service requested, proposed schedule, travel requirements and current worker or candidate capacity.
Does a partnership enquiry create a formal agreement?
No. An enquiry, meeting, referral discussion or exchange of information does not create a formal partnership, contract, exclusivity arrangement or guaranteed referral pathway.
Any ongoing responsibilities, information-sharing process, fees or commercial terms must be confirmed in writing.
Can I request a professional information pack?
Yes. Select “Request a professional information pack” in the enquiry form.
We will provide the current information available about service scope, referral considerations, contact routes and next steps. The information pack does not replace any written service agreement or referral-specific discussion.
